Welcome!

In the National Osteoporosis Foundation Tribute Center you can honor the living or memorialize the deceased in one of several ways.  Tributes can either be in the form of a one time donation to honor or memorialize someone or it can be a special web page that tells the story of a special person and/or your personal commitment to supporting NOF.

If you wish to do something truly special to honor or memorialize someone, you can create or contribute to an existing tribute in that person’s name.  If you create such a web page to honor someone special you can then invite other family members and/or friends to join you with their own testamonial and a contribution to NOF in his or her honor .  If you are interested in a one time gift in honor someone or you wish to create or find a tribute page then click on one of the following buttons:   

By clicking the above button you can make a contribution to honor someone special (e.g., birthday, anniversary, special event, etc.) or pay tribute to a person who has died.  Either way, an appropriate notification will be sent to the honoree or to the family of the deceased.  Additionally, an acknowledgement of your donation will be sent to you.

Clicking the Create a Tribute button above will take you to the pages that you need to complete to create your tribute, which will honor someone special while assisting NOF in raising much needed funds.  After naming your tribute, providing basic information about it and making a $25 minimum donation, you can:

• Tell your story about the person being honored or memorialized.
• Upload a digital photo to be featured on the tribute web page. 
• Provide the street and email address of key people that can be notified when gifts are made to the fund.
• Provide a personal donation as the initial gift to the fund.

After creating your tribute , you can use the NOF Tribute Center to personalize your Tribute Web Page, send an email to your family and friends to ask them to visit your web site, and more.

Clicking the Find a Tribute Page link displays a search page that you can use to locate the tribute web page of someone you know.

Note: If you have previously registered with our site, you should log in now to use existing information from your profile.  If you have not registered, you will be asked to enter your mailing and email addresses to create a User Name/Password login during the fund creation process.
